SC Updates

As most of you know from our provider meetings, Sharon Coleman, SC, is moving to Arkansas. Her last day is next week. Please join us in wishing her and her family well as they embark on this new journey.

With this change, Marta Bagley and Renell Finke have accepted full-time SC roles within our cluster. (Both were previously part-time). In addition, Melissa de Fluiter will begin providing SC services again, too. We will not hire new staff; however, we are adjusting the coverage areas for Goldie, Renell, Marta and Melissa.

The following is the coverage plan for First Steps of North Central Indiana:
Amy Freeman - Fulton, Cass, Miami
Cris Volmer - Grant
Emily Brown - Marshall
Goldie Reynolds - Wabash, Kosciusko (south of US 30 except Winona Lake)
Kristi Scheer - southern Miami, northeast Howard, northwest Grant
Lilia Martinez - Spanish-speaking families
Marta Bagley - Marshall, Kosciusko (north of US 30 and Winona Lake)
Melissa de Fluiter - Starke
Natalie Brookins - Howard
Renell Finke - Cass, Miami, Pulaski
Stephanie Neuhauser - Howard

For Sharon's children who will be turning 3 years through March 2010, Emily will be their SC. All other families will be re-assigned based on the information above. Families are being contacted via phone and mail to notify them of these changes. Providers will be notified from the new SC. If you are unsure of the SC, please contact the SPOE office and the office staff will be able to help you.

The next LPCC and Transition Committee meetings are Thursday, November 19 at 9:30am and 12:30pm respectively.

We will meet at McClure's Orchard in Peru. Lunch will be served at 11:30am for those attending both meetings.

Updates and Reminders

Hello!

*Emily Brown has a new baby boy! Elliot was born on Saturday, July 11th at 9:46AM weighing 8lbs 5oz. Mom and baby are doing well!

*Emily will be on leave until Sept 1st - continue to contact the office for questions about families she serves.

*New Service Coordinators - Marta, Renell and Lilia have successfully completed all SC training and shadowing. Please welcome them!

*Start date of service - your start date is the date listed on the sec 8/change page or MD's signature date - whichever is LATEST. Do not start services until you receive the signed sec 8/change page from the SPOE office.

*Please remember to submit your service start date to the SC for each initial, change, and annual. (Basically, any new authorization)

*Matrix - please make sure your matrix with all required info (phone, fax, email) is current!

*Availability - please update as frequently as your availability changes!

*When making a change to your availability (i.e. changing servicing zip codes or leaving the program), please remember we MUST give families a 10-day notice to any IFSP change (this includes a provider change).
